    This just in, courtesy of Addiction Treatment Forum:

    WASHINGTON, DC -- Journal of Substance Abuse Treatment; Implementation of a clinic policy of client-regulated methadone dosing; April 2001, Vol. 20, No. 3: pp. 225-230 (Robles E, Miller FB, Gilmore-Thomas KK, McMillan DE) -- Methadone maintenance treatment patients significantly reduce their use of illicit opioids when they have a say in their daily methadone dose. Meanwhile, average dose increases are minimal with this approach.

    A six-month interval (baseline) during which methadone doses above 99 mg required individual approval by the clinic's physician was compared with the subsequent
    16-month period in which a policy of patient-regulated methadone dosing with no preset upper limit was implemented. During the later phase, all patients were required to remain at each selected dose for a minimum of 4 days, and standard compliance-based take-home dosing procedures were followed.

    For patients in the study (n=57), the maximum daily methadone dose increased from 165 mg during baseline to 300 mg during the self-regulation period; however, their
    average daily methadone dose increased only from 76.84 mg to 80.04 mg. Monthly percent of opiate-positive urine specimens decreased significantly from 5.26% during baseline to 1.64% during the self-regulated dose period, and use of other drugs remained unchanged. No patient failed to show possession of recalled take-home doses, and no instances of liquid methadone diversion were reported by law enforcement agencies in the area.

    Now available for the first time: the long awaited, unpublished (cleared but never printed by NIDA) Research Monograph on PAIN TREATMENT IN ADDICTS AND OTHERS WHO MAY HAVE HISTORIES OF DEPENDENCE. Had money been available in the NIDA printing budget in the mid-nineties, this would have been Monograph #169. Despite a favorable review by then Assistant Secretary for Health Philip R. Lee, the monograph was deemed a lower priority than other competing print needs at NIDA. In the years since, many participants in the NIDA Technical Review that generated the monograph, as well as other colleagues, have asked Dr. Trachtenberg for the consensus statement that was developed at the meeting and included in the introduction to the monograph. While somewhat dated, much of the material in the monograph is still very relevant. It is unfortunate that the state of the science in the field has not advanced quickly enough to make the monograph completely outdated, However, the reader (and his or her physician) must judge for him/herself which portions may still be useful. This monograph is presented for discussion only and should not be taken as medical advice. Scroll down to the Chronic Pain Section to view the monograph.     The complete text has now been posted here for ALTERNATIVE MEDICINE: EXPANDING MEDICAL HORIZONS (Often called "The Chantilly Report"): A Report to the National Institutes of Health on Alternative Medical Systems and Practices in the United States; Prepared under the auspices of the Workshop on Alternative Medicine, Chantilly, Virginia, September 14-16, 1992. This was published in 1995 by the NIH Office of Alternative Medicine (OAM), while Dr. Trachtenberg was directing it. Originally printed by the Government Printing Office (GPO), and used as a text book in many classes (including Dr. Trachtenberg's class on Alternative & Complementary health Practices at the George Washington University School of Public Health and Health Serivices, this large, orange, soft-cover book has since become very difficult to find and is no longer available from NIH. The book remains a remarkable resource and still fulfils it's original role, as a landmark map of the territory of alternative and complementary health practices (ACHP) in North America. While developed by large non-federal panels of diverse health professionals, and NOT ENDORSED by any official agency, this document was a federal government publication and therefore there is no copyright attached. Readers are reminded not to try any of the therapies discussed in this volume without consulting with a personal physician or other licensed health professional.
